Use of Biphasic Insulin Aspart 30 in Type 2 Diabetes Treatment: Expert Panel Recommendations

The goals of Type 2 diabetes treatment are to eliminate the hyperglycemia resulting from insulin insufficiency and/or insulin resistance, delay beta cell damage/depletion, and prevent other metabolic co-morbidities and complications. In the current treatment algorithms, lifestyle changes (medical nutrition therapy, physical exercise) and oral anti-diabetics are followed by insulin therapy, which is considered a replacement therapy for Type 2 diabetes. Pre-mixed insulin preparations, which are an option for patients with poor blood glucose level control under oral anti-diabetics treatment, have been developed to meet both basal and p . . .randial insulin needs by simulating the physiological changes in insulin levels. The consensus on the necessity of individualizing insulin therapy requires physicians to have a detailed knowledge of the various uses of insulin. Therefore, this comprehensive consensus statement has been prepared by a panel of expert endocrinologists from different regions of Turkey to help physicians use biphasic insulin aspart 30 in suitable patients at the right time. In this statement, expert panel opinions on (a) Recommendations for the appropriate initiation, titration, and intensification of insulin treatment, and (b) The treatment algorithms in initiation, titration, and intensification of biphasic insulin aspart 30 treatment and special conditions specific to changing treatment regimen are presented. Tip 2 diyabet (T2D) tedavisinde amaç, insülinin yetersiz salgısı ve/veya insülin direnci sonucu oluşan hiperglisemiyi ortadan kaldırmak, beta hücre harabiyetini/tükenişini yavaşlatmak, eşlik eden diğer metabolik sorunları düzeltmek ve komplikasyonları önlemektir. Güncel tedavi algoritmalarında yaşam tarzı değişikliği (tıbbi beslenme tedavisi, egzersiz) ve oral antidiyabetikler ile tedavi seçeneklerini izleyen aşamada yer alan insülin, T2D için bir yerine koyma tedavisi olarak değerlendirilmektedir. Hazır karışım insülinler, oral antidiyabetikler ile kan glukoz düzeyleri kontrol altına alınamayan hastaların tedavisinde yer alması önerilen tedavi seçeneklerinden olup, hem bazal hem de prandiyal insülin ihtiyacını fizyolojiye yakın biçimde karşılamak amacıyla geliştirilmişlerdir. İnsülin tedavisinin kişiye özel olması gerektiği konusundaki konsensüs, hekimlerin değişik insülinlerin kullanımları ile ilgili daha ayrıntılı bilgiye sahip olmalarını gerektirmektedir. Bu nedenle, ülkemiz koşullarında bifazik insülin aspart 30’un doğru hastada, doğru zamanda ve doğru biçimde kullanılmasını kolaylaştırmak ve hekimlere yardımcı olmak için Türkiye’nin farklı bölgelerinde görev yapmakta olan deneyimli endokrinologlardan oluşan bir uzman paneli tarafından, bu kapsamlı uzlaşı metni hazırlanmıştır. Background: Precise techniques to find the facial nerve (FN) and recess are lacking. Objectives: We aimed to define incus-spine and incus-FN angles which can be used to localize the FN and recess during mastoidectomy. Material and methods: Thirty adult cadaveric temporal bones were studied. Canal-wall up mastoidectomy with a facial recess approach was performed. The temporal bones and microscope were positioned differently to change the visual angle. The following distances were measured: (1) Short process of the incus (SPI)-FN; (2) Body of the incus-FN. Photographs were taken. Three lines were drawn on the photographs between the S . . .PI, FN, and the spine of Henle. The angles were created and measured. Results: Three of the temporal bones were excluded due to the absence of the spine of Henle and two of them due to the displacement of the SPI. The mean of the incus-spine angle in 25 temporal bones was 90.12 degrees and the mean of the Incus-FN angle was 135.96 degrees. The mean distances of the SPI-FN and body of incus-FN were 4.85 and 9.26 mm, respectively. Effect of the Dexamethasone-Ondansetron Combination Versus Dexamethasone-Aprepitant Combination to Prevent Postoperative Nausea and Vomiting

Objective: Postoperative nausea and vomiting (PONV) is a common problem associated with general anaesthesia. The incidence can be as high as 80% in high-risk patients. Our primary objective was to compare the efficacy of the combination of dexamethasone-ondansetron and dexamethasone-aprepitant in patients undergoing laparoscopic surgery.Methods: Seventy 18 to 60 years old patients scheduled for laparoscopic surgery were included in the study. Sixty-seven patients completed the study. Patients in the dexamethasone-aprepitant group (group DA, n35) received 40 mg of aprepitant orally 1-2 hours before the induction of anaesthesia and 2 . . .mL saline intravenously (iv) within the last 30 minutes of surgery; patients in the dexamethasone-ondansetron group (group DO, n35) received oral placebo identical to aprepitant 1-2 hours before the induction of anaesthesia and 4 mg ondansetron iv within the last 30 minutes of surgery. All patients received 8 mg dexamethasone iv after the induction of anaesthesia. The primary outcome was a complete response (no postoperative nausea, retching and vomiting and no need for rescue antiemetic); the secondary outcomes were the incidence of nausea, retching, vomiting, the need of rescue antiemetic and opioid consumption within 24 hours after surgery.Results: A complete response was not significantly different between the groups (group DO: 67%, DA: 69%) at 24 hours (p0.93). The incidence of PONV and postoperative opioid consumption was similar between the groups.Conclusion: The study was designed to evaluate whether the combination of dexamethasone-aprepitant is better than the combination of dexamethasone-ondansetron regarding the complete response for PONV in patients undergoing laparoscopic surgery. The results however showed that dexamethasone-aprepitant has not improved the complete response for PONV compared to dexamethasone-ondansetron. Amaç: Postoperatif bulantı ve kusma (POBK) genel anestezi sonrası sık karşılaşılan bir sorundur. Yüksek riskli hastalarda insidans yüzde 80 kadar yüksek olabilir. Çalışmamızın amacı laparoskopik ameliyat geçiren hastalarda deksametazon-ondansetron ve deksametazon-aprepitant kombinasyonunun etkililiğini karşılaştırmaktı.Yöntemler: Bu çalışmaya laparoskopik ameliyat planlanan 18-60 yaş arası 70 hasta alındı. 67 hasta çalışmayı tamamladı. Deksametazon aprepitant grubundaki (grup DA, n35) hastalara anestezi indüksiyonundan 1-2 saat önce 40 mg aprepitant ve cerrahinin son 30 dakikasında intravenöz 2 mL serum fizyolojik (iv) uygulandı. Deksametazon-ondansetron grubundaki (grup DO, n35) hastalara anestezi indüksiyonundan 1-2 saat önce aprepitanta benzer oral plasebo ve cerrahinin son 30 dakikasında 4 mg ondansetron iv olarak verildi. Tüm hastalara anestezi indüksiyonundan sonra iv 8 mg deksametazon uygulandı. Birincil sonuç ölçütü tam yanıt idi (postoperatif bulantı, öğürme, kusma ve ek antiemetik ihtiyacı yok); ikincil sonuç ölçütü ise bulantı, öğürme ve kusma insidansı, operasyondan sonraki 24 saat içerisinde ek antiemetik ve opioid tüketimi ihtiyacı idi.Bulgular: Tam yanıt gruplar arasında (grup DO: %67, DA: %69) 24 saatte anlamlı şekilde farklı değildi (p0,93). POBK insidansı ve postoperatif opioid tüketimi gruplar arasında benzerdi.Sonuç: Bu çalışma, laparoskopik cerrahi geçiren hastalarda POBK için tam yanıt elde etmede deksametazon-aprepitant kombinasyonunun deksametazon-ondansetron kombinasyonundan daha iyi olup olmadığını değerlendirmek üzere tasarlanmıştır. Imaging Capsular Block Syndrome with Anterior Segment Optical Coherence Tomography

Purpose: To evaluate patients prediagnosed with capsular block syndrome (CBS) using slit-lamp adapted anterior segment optical coherencetomography (SL-OCT) prospectively.Materials and Methods: The eyes of the patients with decreased vision following phacoemulsifi cation and intraocular lens (IOL) implantationprediagnosed as capsular block syndrome were included. Following ophthalmological examination with pupillary dilation, SL-OCT imagingwas performed.Results: The study included 17 eyes of 17 cases with late onset CBS. The mean period of time between cataract operation and CBS diagnosiswas 66.5 31.6 months ( range 5 to 120 months). . . . The interspace between posterior surface of the IOL and posterior capsule was hyporefl ectivein six cases and hyperrefl ective in other cases. The mean posterior displacement of posterior capsule was 1.02 0.74 mm (ran ge 0.35 to 2.17mm) at the deepest location. Except for three cases all were treated with Neodymium:YAG (Nd:Y AG) laser posterior capsulotomy.Conclusion: Late-onset CBS can be seen years after phacoemulsifi cation surgery and is thought to be occurred by the liquefi ed retained corticalmaterial. Also, this condition may be misdiagnosed as IOL opacifi cation. SL-OCT is a useful and supportive method in the demonstration anddifferential diagnosis of CBS. Amaç: Kapsüler Blok Sendromu (KBS) ön tanısı konulan olguların biyomikroskop ile birleştirilmiş Ön Segment Optik Koherens Tomografi si(ÖS-OKT) ile prospektif olarak değerlendirilmesi.Gereç ve Yöntemler: Fakoemülsifi kasyon ve göz içi lensi (GİL) implantasyonu sonrası görmede azalması olan, KBS ön tanısı koyulan olanhastalar çalışmaya dahil edildi. Pupilla dilatasyonu öncesi ve sonrası yapılan oftalmik muayene ardından ÖS-OKT ile ön segment görüntülemesiyapıldı.Bulgular: Çalışma geç başlangıçlı KBS olan 17 hastayı kapsamaktadır. Katarakt ameliyatı ile KBS tanısı arasında geçen süre ortalama 66.5 31.6 ay (dağılım 5 ve 120 ay) idi. Arka kapsülün posteriorda yer değişimi en derin noktada 1.02 0.74 mm (dağılım 0.35 ve 2.17 mm) olarakölçüldü. GİL arka yüzeyi ve arka kapsül arasındaki boşluk 6 olguda hiporefl ektif, diğerlerinde hiperrefl ektif olarak izlendi. Üç vaka dışındatamamına Neodymium:YAG laser arka kapsulotomi uygulandı.Sonuç: Geç dönem KBS, fakoemülsifi kasyon ameliyatından yıllar sonra görülebilmekte ve likefi ye korteks materyaline bağlı olduğudüşünülmektedir. A Proportional Time Allocation Algorithm to Transmit Binary Sensor Decisions for Target Tracking in a Wireless Sensor Network

In this paper, we study the target tracking problem in a wireless sensor network. A sensor receives a measurement from an energy emitting target and employs binary quantization to the received measurement to generate its decision. A sinusoidal waveform with a certain duration is then used to transmit the sensor decision to the fusion center (FC). All sensor decisions are transmitted to the FC over erroneous wireless channels based on a time division multiple access scheme. We introduce the proportional time allocation (PTA) algorithm where at each time step of tracking, PTA jointly determines the sensors binary quantization threshol . . .ds and their time allocations devoted for the transmissions of binary sensor decisions. Simulation results show that, PTA optimally and dynamically distributes the available transmission time among the sensors near the target so that the decisions of such sensors become less subject to channel errors, and turns off the non-informative sensors located far away from the target. Make-or-buy decision is an important factor affecting the profitability of the firms in all sectors. The goal of this study is to propose a model for firms in engineering design services sector for make-or-buy decisions. A survey was conducted to determine the importance percentages given in an engineering company in make-or-buy decisions and a model was developed. The results of the case study show intriguing clusters of company personnel.
