The effect of N G -nitro L-arginine methyl ester on colonic anastomosis after increased intra-abdominal pressure

Background: Intestinal ischemia-reperfusion (I/R) is associated with augmented nitric oxide (NO) production. Increased intra-abdominal pressure (IAP) during surgical pneumoperitoneum (P) facilitates I/R injury. We previously demonstrated decreased strength and healing of colocolic anastomoses after high IAPs. The effect of an NO synthase inhibitor, N G -nitro-arginine methyl ester (l-NAME), on anastomoses realized in colonic tissue exposed to high IAPs was investigated in this study, a randomized, controlled, and experimental study with blind outcome assessment. Method: Fifty Wistar-albino rats were randomized to five groups; all un . . .derwent colocolic anastomosis. P was maintained for 60 min at IAPs of 14, 20, 25, and 30 mmHg in study groups 1, 2, 3, and 4, respectively; P was preceded by intraperitoneal l-NAME (2.5 mg/kg) and followed by anastomosis. The control group was not subjected to IAP or l-NAME. Results: Anastomosis bursting pressure (ABP) values and histopathological findings were determined on the 7th-14th postoperative days. The ABPs of groups 3-4 were significantly lower than the others. Groups 1-2 had results similar to controls. Histopathological findings of the groups were consistent with their ABPs. Conclusion: Administration of a 2.5-mg/kg intraperitoneal l-NAME dose was found to provide a beneficial role, implying a role in impaired anastomotic healing after IAPs of 14 and 20 mmHg. © 2006 Springer-Verlag Daha fazlası Daha az

Optical studies of semiconductor Quantum Dots

Optical absorption (ABS), steady-state photoluminescence (PL), resonant Raman, and photoabsorption (PA) spectroscopies are employed to study quantumsize effects in II-VI semiconductor quantum dots (QDs) grown in glass samples. We observe a size-dependent shift in the energetic position of the first exciton peak and have examined the photoinduced evolution of the differential absorption spectra. The Raman shifts of the phonon modes are employed to monitor stoichiometric changes in the composition of the QDs during growth. Two sets of glass samples were prepared from color filters doped with CdSxSe1-xand ZnxCd1-xTe. We analyze the opt . . .ical properties of QDs through the ABS, PL, resonant Raman, and PA spectroscopies. The glass samples were prepared from commercially available semiconductor doped filters by a two-step thermal treatment. The average size of QDs is estimated from the energetic position of the first exciton peak in the ABS spectrum. A calculation based on a quantized-state effective mass model in the strong confinement regime predicts that the average radius of QDs in the glass samples ranges from 2.9 to 4.9 nm for CdTe and from 2.2 to 9.3 nm for CdS0:08Se0:92. Leuconostoc mesenteroides subsp. cremoris is an obligate heterolactic fermentative lactic acid bacterium that is mostly used in industrial dairy fermentations. The phosphoketolase pathway (PKP) is a unique feature of the obligate heterolactic fermentation, which leads to the production of lactate, ethanol, and/or acetate, and the final product profile of PKP highly depends on the energetics and redox state of the organism. Another characteristic of the L. mesenteroides subsp. cremoris is the production of aroma compounds in dairy fermentation, such as in cheese production, through the utilization of citrate. Considering its importan . . .ce in dairy fermentation, a detailed metabolic characterization of the organism is necessary for its more efficient use in the industry. To this aim, a genome-scale metabolic model of dairy-origin L. mesenteroides subsp. cremoris ATCC 19254 (iLM.c559) was reconstructed to explain the energetics and redox state mechanisms of the organism in full detail. The model includes 559 genes governing 1088 reactions between 1129 metabolites, and the reactions cover citrate utilization and citrate-related flavor metabolism. The model was validated by simulating co-metabolism of glucose and citrate and comparing the in silico results to our experimental results. Model simulations further showed that, in co-metabolism of citrate and glucose, no flavor compounds were produced when citrate could stimulate the formation of biomass. Significant amounts of flavor metabolites (e.g., diacetyl and acetoin) were only produced when citrate could not enhance growth, which suggests that flavor formation only occurs under carbon and ATP excess. The effects of aerobic conditions and different carbon sources on product profiles and growth were also investigated using the reconstructed model. The bilateral trade flows of the EU in the presence of structural breaks

This paper examines the bilateral trade dynamics of the EU with its major trade partners. The impacts of the real exchange rate and real income on the trade balance of the EU have been investigated in the presence of structural breaks. The empirical analysis includes ten major trade partners of the EU for 1980–2012, on a quarterly basis. The paper applies the Kejriwal and Perron (J Econom 146(1):59–73, 2008, J Bus Econ Stat 28(4):503–522, 2010a) structural break test to determine the presence of structural breaks in series and to estimate the parameters of the model. In order to test the cointegration relationships of series, three . . .different cointegration techniques were applied to the data. First, the Gregory and Hansen (Oxf Bull Econ Stat 58(3):555–560, 1996) cointegration test allows for one structural shift; then, for cases where two breaks were detected, the Hatemi-J (Empir Econ 35(3):497–505, 2008) cointegration test was employed. Finally, for countries where more than two breaks are detected, the Maki (Econ Model 29(5):2011–2015, 2012) cointegration test was applied, which allows for an unknown number of breaks. This paper presents evidence to show that income is a determining factor in the bilateral trade of Europe rather than a real exchange rate. However, allowance of structural breaks in cointegration tests reveals the absence of cointegrating relations in export demand equations of China and Norway and in import demand equations of Japan and Norway. Hydroxychloroquine improves dry eye symptoms of patients with primary Sjogren's syndrome

The objective of the study is to investigate the effect of hydroxychloroquine (HCQ) on subjective and objective parameters of dry eye in patients with primary Sjogren's disease and to evaluate the association of tear fluid B-cell activating factor (BAFF) level with the response. Thirty-two patients with primary Sjogren's disease were enrolled in this prospective study. All patients included in the study completed at least a 48-month run-in period of using hydroxychloroquine. Patients were then instructed to drop the treatment for 3 months. Baseline and post cessation of treatment (baseline and 3 months) evaluations included, subject . . .ive symptom scoring, fluorescein and lissamine green staining, Schirmer's test, tear break-up time (BUT) and tear fluid BAFF assessments. Significant worsening was observed in, tear break up-time (TBUT) (7.9 ± 3.4 vs. 5.9 ± 2.9, P < 0.001) lissamine green of staining of the ocular surface (1.3 ± 0.9 vs. 1.8 ± 0.8, P < 0.01) and corneal fluorescein staining scores (2.2 ± 2.1 vs. 4.6 ± 3.3, P < 0.003) between on and off HCQ treatment, respectively. Similarly, gritty sensation and burning sensation were significantly changed at week 12 compared to baseline evaluation (1.18 ± 1.02 vs. 1.7 ± 1.05, P < 0.007 and 1.1 ± 1.0 vs. 1.6 ± 1.2, P < 0.0, respectively). Disease duration significantly correlated with baseline OSDI (r = 0.38, P < 0.04) and the average daily use of artificial tears (r = 0.36, P < 0.04). The mean BAFF levels were 0.8 ± 0.5 and 4.0 ± 0.7 ng/ml for baseline and week 12 evaluation, respectively (P < 0.0001). Hyper-heuristics for performance optimization of simultaneous multithreaded processors

In Simultaneous Multi-Threaded (SMT) processor datapaths, there are many datapath resources that are shared by multiple threads. Currently, there are a few heuristics that distribute these resources among threads for better performance. A selection hyper-heuristic is a search method which mixes a fixed set of heuristics to exploit their strengths while solving a given problem. In this study, we propose learning selection hyper-heuristics for predicting, choosing and running the best performing heuristic. Our initial test results show that hyper-heuristics may improve the performance of the studied workloads by around 2%, on the aver . . .age. The peak performance improvement is observed to be 41% over the best performing heuristic, and more than 15% over all heuristics that are studied. Pneumatization degree of the anterior clinoid process: A new classification

The objective of this study is to determine the incidence and degree of anterior clinoid process pneumatization, in addition highlighting to their clinical significance. Multidetector-row CT scans of the skull base were reviewed in 648 subjects between 2007 and 2008. The presence of pneumatized anterior clinoid process and its degree were studied and documented. These datas were istatistically analyzed. Pneumatization of the ACP was found in 62 of 648 patients (9.6%) including 32 (51.6%) men and 30 (48.4%) women. The age of these patients ranged from 21 to 82 years (mean, 41±15.7 years). Pneumatization of the ACP occurred only on th . . .e left side in 14 cases (22.6%), only on the right side in 11 cases (17.7%), and bilaterally in 37 patients (59.7%). ACP pneumatization Type I, in which less than 50% of the ACP is pneumatized, was found in 47 of 124 sides (38%), Type II, in which more than 50% but not totally pneumatized ACP, was found in 28 of 124 sides (22.6%), and Type III, in which the ACP is totally pneumatized, was found in 22 of 124 sides (17.7%). The incidence of Type I in the general population was 6.6%, Type II was 3.5%, and Type III was 2.5%. Radiologically recognizing the degree of ACP pneumatization is important in decreasing the incidence of surgical complications during anterior clinoidectomy. The Post-Ureteroscopic Lesion Scale (PULS): A multicenter video-based evaluation of inter-rater reliability

Abstract: Purpose: The Post-Ureteroscopic Lesion Scale (PULS) offers a simple grading system for the description of ureteral lesions after ureteroscopy. In this article, we present the results of a video-based multicenter evaluation of the inter-rater reliability of clinically important PULS grades 0-3. Methods: Video sequences at the end of ureteroscopy (final passage) were recorded for 100 consecutive patients at a single institution and assessed by experienced urologists (n = 20) and senior residents (n = 17) at 19 international centers. The cohort included only patients with lesions grades 0-3 (with grades 2 and 3 subsumed as 2 . . .+ since distinction is defined by an extravasation of contrast medium in fluoroscopy). The gradings were evaluated for inter-rater reliability and in terms of simplicity, validity, comprehensibility, reproducibility, and usefulness. Results: Overall, inter-rater reliability was high (Kendall's W = 0.69, p < 0.001) and was comparable between specialists (Kendall's W = 0.69, p < 0.001) and residents (Kendall's W = 0.71, p < 0.001). The matched ratings showed grade 0 in 43.0 % of patients and grades 1 or 2 + in 44.0 and 13.0 % of patients, respectively. Results of the questionnaires indicated a high degree of acceptance, with an overall rating of 1.76 (1.64-1.93 for different items, scale 1-6). Conclusions: Inter-rater reliability of the endoscopically assessable PULS was high among urologists with different levels of experience in different countries worldwide. The validated PULS system may be used for standardized reporting of ureteral lesions/injuries after ureteroscopy. History-based predictive instruction window weighting for SMT processors

In a Simultaneous Multi-Threaded (SMT) processor environment, threads share datapath resources, and resource allocation policy directly affects the throughput metric. As a way of explicit resource management, resource requirements of threads are estimated based on several runtime statistics, such as cache miss counts, Issue Queue usage and efficiency metrics. Controlling processor resources indirectly by means of a fetch policy is also targeted in many recent studies. A successful technique, Speculative Instruction Window Weighting (SIWW), which speculates the weights of instructions in Issue Queue to indirectly manage SMT resource . . .usage, is recently proposed. SIWW promises better peformance results compared to the well-accepted ICOUNT fetch policy. In this study, we propose an alternative fetch policy that implements SIWW-like logic using a history-based prediction mechanism, History-based Predictive Instruction Window Weighting (HPIWW), avoiding any types of speculation hardware and its inherent complexity.
