Use of Biphasic Insulin Aspart 30 in Type 2 Diabetes Treatment: Expert Panel Recommendations

The goals of Type 2 diabetes treatment are to eliminate the hyperglycemia resulting from insulin insufficiency and/or insulin resistance, delay beta cell damage/depletion, and prevent other metabolic co-morbidities and complications. In the current treatment algorithms, lifestyle changes (medical nutrition therapy, physical exercise) and oral anti-diabetics are followed by insulin therapy, which is considered a replacement therapy for Type 2 diabetes. Pre-mixed insulin preparations, which are an option for patients with poor blood glucose level control under oral anti-diabetics treatment, have been developed to meet both basal and p . . .randial insulin needs by simulating the physiological changes in insulin levels. The consensus on the necessity of individualizing insulin therapy requires physicians to have a detailed knowledge of the various uses of insulin. Therefore, this comprehensive consensus statement has been prepared by a panel of expert endocrinologists from different regions of Turkey to help physicians use biphasic insulin aspart 30 in suitable patients at the right time. In this statement, expert panel opinions on (a) Recommendations for the appropriate initiation, titration, and intensification of insulin treatment, and (b) The treatment algorithms in initiation, titration, and intensification of biphasic insulin aspart 30 treatment and special conditions specific to changing treatment regimen are presented. Tip 2 diyabet (T2D) tedavisinde amaç, insülinin yetersiz salgısı ve/veya insülin direnci sonucu oluşan hiperglisemiyi ortadan kaldırmak, beta hücre harabiyetini/tükenişini yavaşlatmak, eşlik eden diğer metabolik sorunları düzeltmek ve komplikasyonları önlemektir. Güncel tedavi algoritmalarında yaşam tarzı değişikliği (tıbbi beslenme tedavisi, egzersiz) ve oral antidiyabetikler ile tedavi seçeneklerini izleyen aşamada yer alan insülin, T2D için bir yerine koyma tedavisi olarak değerlendirilmektedir. Hazır karışım insülinler, oral antidiyabetikler ile kan glukoz düzeyleri kontrol altına alınamayan hastaların tedavisinde yer alması önerilen tedavi seçeneklerinden olup, hem bazal hem de prandiyal insülin ihtiyacını fizyolojiye yakın biçimde karşılamak amacıyla geliştirilmişlerdir. İnsülin tedavisinin kişiye özel olması gerektiği konusundaki konsensüs, hekimlerin değişik insülinlerin kullanımları ile ilgili daha ayrıntılı bilgiye sahip olmalarını gerektirmektedir. Bu nedenle, ülkemiz koşullarında bifazik insülin aspart 30’un doğru hastada, doğru zamanda ve doğru biçimde kullanılmasını kolaylaştırmak ve hekimlere yardımcı olmak için Türkiye’nin farklı bölgelerinde görev yapmakta olan deneyimli endokrinologlardan oluşan bir uzman paneli tarafından, bu kapsamlı uzlaşı metni hazırlanmıştır. Bean diseases caused by different pathogens reduce important yield and quality of beans in different bean growing regions in both Turkey and the other bean producing countries. In the present study, bacterial pathogens in the species of Pseudomonas savastanoi pv. phaseolicola (Burkholder) Garden et al. and Xanthomonas axonopodis pv. Phaseoli (Smith) Vauterin et al. causing economically important disease on bean plants growing in the commercial fields of Erzurum and Erzincan provinces located in the Eastern Anatolia region of Turkey has been isolated and identified. Diabetes mellitus and phytotherapy in Turkey

This study reports a literature review aimed to analyse various studies related to the use of phytotherapy in diabetes mellitus in Turkey in order to provide additional information for healthcare professionals. The incidence of Diabetes Mellitus is rising and many of the diabetics frequently use herbal treatments along with modern medical treatment for glycaemic control and/or improve their well-being. Several electronic databases (such as Medline and Pubmed) were searched for 1990-2010 period (till May, 2010) and 33 related articles were analysed. Many studies-mostly animal trials- have been conducted in this field. Among the herbs . . . most-commonly used along with modern medical therapies and also in folkloric medicine, we searched for bitter melon, cinnamon, fenugreek, olive leaf, black seed and white mulberry. Studies conducted in this field have produced conflicting results and, the necessity to conduct randomized, placebo-controlled clinical human studies to develop new drugs from herbs, as in the case of metformin, still remains important. Besides, further studies are required to address the issues of standardization and quality control of existing preparations. Assessment of methods for determining the impurity concentration in mercury cells

The uncertainty arising from chemical impurities is the principle contribution in the uncertainty budget of primary level temperature measurements. Impurities in any substance generally decrease the freezing (or triple) point temperature of a substance, and their influence is governed primarily by their behavior at low concentrations in the host material. The depression in temperature due to impurities is theoretically expressed by Raoult's law which, at final analysis, expresses the linearity between ?T (T observed-T pure) and the inverse of the melted fraction (1/F). Recently, TUBITAK UME carried out a new project on the construct . . .ion of new reference mercury fixed-point cells. Within the scope of this study, three different sets of mercury cells with different purity values were fabricated. Three methods were employed to assess the impurity concentration in the cells. The first method is known as the mole fraction sum of impurity components, and the chemical assays form the basis for this kind of assessment. The second method of evaluation is based on a 1/F versus ?T curve, and the slope values obtained from these curves are important. The final method is to directly compare the new cells with a national (or reference) standard mercury cell. The effect of low-level laser therapy as an adjunct to non-surgical periodontal treatment on gingival crevicular fluid levels of transforming growth factor-beta 1, tissue plasminogen activator and plasminogen activator inhibitor 1 in smoking and non-smoking chronic periodontitis patients: A split-mouth, randomized control study

Background and Objective: This study aimed to investigate the effects of low-level laser therapy (LLLT) as an adjunct to scaling and root planing (SRP) on smoking and non-smoking patients with chronic periodontitis. Material and Methods: The study was conducted using a split-mouth design with 30 patients with chronic periodontitis (15 smokers, 15 non-smokers) and 30 healthy individuals matched for age, sex and smoking status as controls. Gingival crevicular fluid samples were collected before periodontal treatment (baseline) and during follow-up visits on days 7, 14 and 30. Gingival crevicular fluid transforming growth factor (TGF)-ß1, tissue plasminogen activator (tPA) and plasminogen activator inhibitor 1 (PAI-1) levels were measured using enzyme-linked immunosorbent assay. Electrochemically designed interfaces: Electrical properties and morphology of micro-nanostructured Titania implant surfaces

Designing Titanium implant interfaces is essential not only for the prevention of corrosion, but also for the promotion of osteointegration. Surface morphology and surface charge are crucial determinants in the construction of biocompatible osteoconductive orthopedic and dental implants. Thus, generation of surfaces with optimal topographic and electrical properties for cellular growth has been aimed. In this study, surface chemistry and morphology of cp Titanium plates were modified by anodization at 30 V for 60 min, in dual acidic solution (48 wt% H2SO4-18 wt% HCl) at 40 °C and in alkaline solution (5 M KOH) at 40°, 60° and 80 °C. . . . Titanium surfaces were characterized for morphology, crystallography, wettability, bulk resistivity, corrosion resistance, surface conductivity and capacitance. Morphological characterization using FE-SEM revealed micro-to-nanoscale porosity, with nanosized pores formed within the micropores, generating a hierarchically arranged micro-nano network on Titanium surfaces anodized under all thermal conditions. Hydrophilicity decreased with increasing alkaline anodization temperature (control, 100 ± 0.7° 40 °C, 23.3 ± 3.0° 60 °C, 43.5 ± 2.1° 80 °C, 55.3 ± 2.3°). The XRD analysis displayed only Ti peaks at 34.5°, 38°, 40°, 53° and 63° 2?. Increasing alkaline anodization temperature modified the electrical properties of Titanium plates. Mott-Schottky analysis displayed an increase in capacitance as indicated by steeper slopes and linearity extending over a greater potential range with increasing temperature. Corrosion current density (Icorr) was experimentally determined as 1.05 × 10- 7A/cm2, 5.95 × 10- 8A/cm2, 3.79 × 10- 9A/cm2, 3.92 × 10- 9A/cm2 while charge carrier density was calculated as 3.44 × 1020/cm3, 3.50 × 1019/cm3, 2.05 × 1019/cm3, 1.01 × 1019/cm3 respectively for the control, 40 °C, 60 °C and 80 °C samples. All things considered, samples modified in 5 M KOH solution at 60 °C displayed optimal micro-to-nanoporosity, lowest corrosion current density and anodic voltage. The electrical properties of modified Titanium surfaces have indicated that it may be more suitable to use Potassium hydroxide in the anodization of biomedical implants which require a biocompatible, osteoconductive topography, high capacitance and high corrosion resistance. © 201 Daha fazlası Daha az

Wealth of information gleaned from decades of high-impact research work; scientists have disentangled the complicated web of versatile regulators that underlie cancer development and progression. Use of structural biology approaches and functional genomics have helped us to gain new insights into complex nature of cancer, and it is now clear that genetic/epigenetic mutations, overexpression of oncogenes, inactivation of tumor suppressors, loss of apoptosis, and versatility of protein binding partners have contributory roles in carcinogenesis and metastatic spread. It is becoming progressively more understandable that reprogramming o . . .f gene expression during and nontranscriptional changes during cancer development and progression are initiated and controlled by deregulated signal transduction cascades, all of which collectively create an incalculable complexity. Data obtained through preclinical and clinical trials revealed that alterations in the targeted oncogenes and other downstream, and parallel pathways played a central role in the development of resistance against different therapeutics. Phytochemicals have regained limelight, and different natural products are currently being tested for efficacy in preclinical studies. Apigenin, a plant-derived flavonoid has considerable pharmacological value and is reportedly involved in the regulation of different signaling cascades. In this review, we have attempted to summarize rapidly evolving understanding of molecular biologists and pharmacologists about the potential of apigenin in the regulation of deregulated signaling pathways in different cancers. We have emphasized on the regulation of WNT/-catenin and janus kinase/signal transducers and activators of transcription (JAK-STAT) pathways. We also comprehensively discuss how apigenin restored apoptosis in tumor necrosis factor-related apoptosis-inducing ligand (TRAIL)-resistant cancers. The review also gives a snapshot of microRNAs (miRNAs) that regulate wide-ranging biological processes, and it is now clear that each miRNA can control hundreds of gene targets. Apigenin was noted to upregulate miR-520b and miR-101 in different cancers to inhibit tumor growth. Moreover, apigenin-induced apoptotic rate was significantly higher when used in combination with miR-423-5p inhibitors or miR-138 mimics.
