SILS incisional hernia repair: Is it feasible in giant hernias? A report of three cases

Aim. Three incisional ventral abdominal wall hernias were repaired by placing a 20 30cm composite mesh via single incision of 2cm. Methods. All three cases had previous operations and presented with giant incisional defects clinically. The defects were repaired laparoscopically via single incision with the placement of a composite mesh of 20 30cm. Nonabsorbable sutures were needed to hang and fix the mesh only in the first case. Double-crown technique was used in all of the cases to secure the mesh to the anterior abdominal wall. Results. The mean operation time was 120 minutes. The patients were mobilized and led for oral intake at . . . the first postoperative day. No morbidity occurred. Conclusion. The determination of airborne fungal flora of two different hospitals in Istanbul (Turkey)

The current study was carried out on monthly fungal flora of Marmara University Hospital and Goztepe Education and Research Hospital in Istanbul between 2005 and 2006. The Petri-plate method based on gravity was used for isolation. A total of 175 fungal colonies were isolated from Marmara University Hospital and the maximum isolation was in May and the minimum isolation was in February. Species isolated from Marmara University Hospital; are Cladosporium (42.9%), Alternaria (18.9%), Penicillium (12.6%), Aureobasidium (11.4%), Aspergillus (6.9%), Mycelia Sterilia (4.6%), Scopulariopsis (1.7%), Rhizopus (0.6%) and Ulocladium (0.6%). A . . .total of 182 fungal colonies were isolated in Goztepe Education and Research Hospital and the maximum isolation was in May and the minimum isolation was in January. Isolated species are; Cladosporium (32.4%), Alternaria (24.2%), Penicillium (15.9%), Aureobasidium (9.3%), Aspergillus (7.1%), Scopulariopsis (3.8%), Ulocladium (3.3%), Mycelia Sterilia (2.7%), Rhizopus (0.5%) and Paecilomyces (0.5%). A number of fungal species with pathogenic potential were encountered among the species isolated in this study. Granules of brushite and octacalcium phosphate from marble

Brushite (DCPD, dicalcium phosphate dihydrate, CaHPO4· 2H2O) and octacalcium phosphate (OCP, Ca8(HPO 4)2(PO4)4·5H2O) granules with millimeter sizes were prepared by using marble (calcium carbonate) granules as the starting material. The method of this study simply comprised of soaking the marble granules in aqueous solutions containing phosphate and/or calcium ions at temperatures between 20° and 37°C. This process did not cause any size change between the initial marble and final brushite or octacalcium phosphate granules. Such DCPD and OCP granules could be useful in maxillofacial, dental, and orthopedic void/bone defect filling a . . .nd grafting applications. An Unusual Cause of a Cystic Lesion with an Osteochondral Defect in the Talus Intraosseous Lipoma

An intraosseous lipoma is a rare benign bone lesion that proliferates from mature lipocytes. It occurs most frequently in the lower limb, particularly in the calcaneus. The talus is an unusual location for this rare lesion. A review of the literature produced only two reports with talar intraosseous lipomas under the name of intraosseous lipomatosis, which described multiple lipomas in different areas. We describe a 38-year-old male patient who had an isolated intraosseous lipoma with an osteochondral defect in the talus and was treated with autologous osteochondral graft transplantation by medial malleolar osteotomy. He could walk . . .with full weightbearing without any assistance at the end of 12 months. Intraosseous lipoma localized in the talus may be confused radiologically with other bone lesions, especially with unicameral bone cyst, if it is associated with an osteochondral defect. Carcinosarcoma of the bladder: Case report and review of the literature

Primary osteosarcomas of the bladder account for about 0.04% of bladder neoplasms. Most of the patients in the literature expired within 6 months and, in almost all of the cases in the literature, radical cystectomy with postoperative chemotherapy was the treatment choice. A 79-year-old gentleman presented with gross hematuria. Cystoscopy demonstrated a 2- to 3-cm tumour along the lateral wall of the bladder. The tumour was resected incompletely via initial transurethral resection of bladder tumour (TURBT), and a second TURBT was subsequently performed to fully resect the residual mass. Surgical pathology from these 2 resections rev . . .ealed osteosarcoma with invasion into the muscularis propria. A cystoprostatectomy was performed and final pathologic specimen revealed high-grade CIS without evidence of residual osteosarcoma. Postoperatively, the patient did not receive chemotherapy or radiation and currently remains disease-free 2 years post-radical cystectomy. Only 33 well-documented cases of primary osteosarcoma of the bladder have been reported to date. This study proposes a new methodology for determining the relationship between child-directed speech and child speech in early acquisition. It illustrates the use of this methodology in investigating the relationship between the morphological richness of child-directed speech and the speed of morphological development in child speech. Both variables are defined in terms of mean size of paradigm (MSP) and estimated in a set of longitudinal spontaneous speech corpora of nine children and their caretakers. The children are aged 1;3-3;0, acquiring nine different languages that vary in terms of morphological richness. Emotional intelligence and teacher efficacy: A study of Turkish EFL pre-service teachers

This study investigated the relationship between emotional intelligence and teacher efficacy among 90 English language pre-service teachers from a university in Turkey. Data sources included Tschannen-Moran and Woolfolk-Hoy's Teachers' Sense of Efficacy Scale and Reuven Bar-On's Emotional Quotient Inventory. The findings indicated that Turkish EFL pre-service teachers felt more efficacious in managing the class rather than in making the class enjoyable. The findings also showed that the pre-service teachers scored highest in the stress tolerance and assertiveness competencies, but lowest in independence and self-regard in terms of e . . .motional intelligence (EQ). Findings revealed a significant, positive relationship between EQ and pre-service teachers' efficacy. The greatest correlation is between the interpersonal EQ subscale and the engagement efficacy subscale. This article focuses on the plight of the Jews in Turkey during the Second World War, with the intention of analysing specific historical events through the lenses of leading theories of nationalism. First we review recent developments in historiography that contribute the framework for understanding both the hermeneutical possibilities and limitations when addressing historical texts. This paper is going to be centred on how learners' autonomy can be fostered. In the literature part of this paper it is stated that the development autonomous learning plays a crucial role in learning process. Autonomy is about how people control their own lives, so when teaching/learning process is taken for granted, teachers create an environment that the students feel themselves autonomous. So autonomy is analyzed both from the teachers' and students' perspective. It analyzes whether the students are independent or not throughout the learning process. The purpose of this study was to investigate the transformation of brushite (dicalcium phosphate dihydrate, DCPD, CaHPO4 2H2O) powders at 36.5 C in DMEM (Dulbecco's Modified Eagle Medium) solutions. Two sets of brushite powders with different particle shapes were synthesized to use in the above DMEM study. The first of these brushite powders was prepared by using a method which consisted of stirring calcite (CaCO3) powders in a solution of ammonium dihydrogen phosphate (NH4H2PO 4) from 6 to 60 minutes at room temperature. These powders were found to consist of dumbbells of water lily-shaped crystals. The second one of the brushite p . . .owders had the common flat plate morphology. Both powders were separately tested in DMEM-immersion experiments. Monetite (DCPA, CaHPO 4) powders were synthesized with a unique water lily morphology by heating the water lily-shaped brushite crystals at 200 C for 2h. Brushite powders were found to transform into octacalcium phosphate (OCP, Ca 8(HPO4)2(PO4)4 5H 2O) upon soaking in DMEM (Dulbecco's Modified Eagle Medium) solutions at 36.5 C over a period of 1 day to 1 week.
