Aim: When selecting a graft for coronary artery bypass surgery (CABG), it is necessary to consider the quality of life and life expectancy of the patient besides ensuring complete revascularization. Use of multiple artery grafts confers the advantage over venous grafts because of their longer patency rates. The aim of this study is to evaluate our experiences in the use of arterial grafts and draw attention to the use of multiple artery grafts. Accordingly, coronary bypass surgeries that we have performed by using multiple arterial grafts were reviewed in our study. Material and Method: Between January 2017 and December 2017, 10 pat . . .ients (8 males average age: 52.75 2 females average age: 61) had undergone CABG surgery by using multiple arterial grafts. We used the left internal mammary artery (LIMA) and radial artery (RA) in 2 patients, LIMA and right internal mammary artery (RIMA) in 6 patients and LIMA-RIMA and radial artery in 2 patients. In 2 of these patients, a T-graft was constructed by anastomosing the proximal end of the free RIMA to the side of the attached LIMA. In all cases, the radial artery was proximally anastomosed to the aorta. All patients were followed up in the intensive care unit for 2 days. Patients were discharged from the hospital in 5-6 days on average. Results: Patients were evaluated in terms of operative mortality, cross clamp time, intensive care and hospitalization period, pain in the incision area, management of post-extubation saturation levels, postoperative complications, recurrent angina, myocardial infarction (MI) and reoperation. No complications were reported during the early period of our evaluations. Although low levels of saturation were detected during this time in patients for whom bilateral IMA was used, saturation increased in the following days with no complications. No sternal dehiscence or infections were observed in patients. Non-steroidal anti-inflammatory (NSAI) and paracetamol anti-analgesics were given to all patients. For measurement of incision area pain, a pain index test was implemented. Discussion: In patients, whose venous grafts are used following CABG, early period graft thromboses lead to repetitive interventional operations and increase in re-operations. Although the superiority of CABG operation versus stent operation is accepted for multiple vein patients, many cardiologists and patients carry objections against the repetitive interventional operations that may arise due to early graft failure and the consequent increase in early period mortality and morbidity risk. We believe that we can overcome these risks by use of multiple artery grafts which allow for a higher patency rate over the long term Daha fazlası Daha az

The UNHCR was established as a subsidiary body by the resolution of the UN General Assembly in 1949 in Geneva. The functions of the UNHCR are defined in the Statute which was adopted in 1950 and in various resolutions subsequently adopted by the UN General Assembly. In accordance with the Statute, the work of the High Commissioner is humanitarian and social and of an entirely non-political character. In this study, the establishment and the scope of missions of the UNHCR which has been operating in Turkey since 1960 are firstly explained. Then the host country agreement between Turkey and the UNHCR which was made in 2016 will be exa . . .mined. In this context, the reasons for making host country agreement; rights immunities, privileges and facilities provided for the UNHCR under this agreement will be described. In this study, the provisions of the treaties and legal arrangements concerning cooperation with the UNHCR from past to the present will also be summarized. The role of the UNHCR in Turkey has been changed since 2013. The factors leading such a change will be identified and the results of the changing role of the UNHCR will be evaluated. Finally, the determinations and proposals made in this study will be stated in the conclusion part Daha fazlası Daha az

Aims: The aim of this study was to compare the effect of different restoration techniques on fracture resistance of endodontically treated teeth with different wall thicknesses. Materials and Methods: Extracted and endodontically treated 210 premolars were randomly divided into three thickness groups [2 mm (A), 1.5 mm (B), and 1 mm (C)] and, each group was further divided into seven restoration subgroups (n = 10): direct composite (control) (K), composite with fiber on cavity floor (KT), composite with fiber on occlusal level (KO), fiber post and composite (FP), inlay (L), fiber on cavity floor and inlay (LT), and inlay and fiber on . . . occlusal level (LO). Fracture test was performed, and data were compared with Kruskal-Wallis and Mann-Whitney U tests (P < 0.05). Results: There were no differences between the subgroups in A and C statistically (P > 0.05). However, in B, KO subgroup showed statistically higher values (P = 0.039). Conclusion: Wall support of 2 mm was adequate, and support of 1 mm was completely insufficient. When the wall thickness was 1.5 mm, direct restoration with fiber at the occlusal level significantly improved resistance. © 2019 Nigerian Journal of Clinical Practic Daha fazlası Daha az

Sexual and gender minority (SGM) medical students and physicians are exposed to bias in professional contexts. One strategy for promoting SGM visibility and inclusion within medicine is the development of institutional OutLists, which are online, opt-in lists of SGM-identified individuals affiliated with an academic institution. We present the first quantitative evaluation of publicly accessible OutLists at medical institutions in the United States, Canada, and Europe. Nineteen OutLists were identified in the United States; no OutLists were identified in other countries. All OutLists in the United States were identified at allopathi . . .c institutions with no institutional religious affiliation. Clinicians in high-prestige specialties and more senior clinicians were underrepresented on OutLists. A state-level measure of SGM equality predicted presence of OutLists within the state (odds ratio 1.429, p = .047) but was not associated with the total number of individuals on OutLists. Future research would benefit from incorporating qualitative methodologies to explore the effectiveness of OutLists and the individual experiences of participants in these lists Daha fazlası Daha az

The aim of the present study was to figure out whether university students’ learning approaches were shaped via their achievement goal orientations, academic self-efficacy and hope or not. The other objective was to examine if these psychological constructs varied in accordance with the demographic variables including gender, age and class level. 332 undergraduates from two different universities who were in the year of junior and senior participated in the study. The Achievement Goal Orientations Scale, the Academic Self-Efficacy Scale, the Dispositional Hope Scale and the Demographic Form were utilized to reveal the predictive pow . . .er of these constructs on their learning attitudes measured by the Revised Two-Factor Study Process Questionnaire. Multiple linear regression analysis indicated that learning goal orientation was a pivotal predictor of both deep and surface approach to learning. Academic self-efficacy and hope were the crucial precursors of deep approach while performance-avoidance goal inclination was a considerable predictor of surface approach. Independent samples t-test analysis displayed that the female undergraduates were superior to the male ones in terms of the learning goal tendency. And the students (20 to 22 aged) demonstrated higher scores on the same variable than the other ones (23 to 25 aged). On the basis of class level, there were no significant differences in the scores of achievement goal orientations, academic self-efficacy, hope and learning approaches. The results pointed out the fact that such concepts pertinent to an undergraduate’s academic performance could be viewed as distinctive features engendering different learning attitudes toward scholastic training. © 2018 Eurasian Society of Educational Research. In a Wireless Sensor Network (WSN), system resources, such as the total node energy, need to be utilized effectively while executing the task of interest. Since a number of sensor nodes may be needed in various type of applications, it is also important to develop each sensor node in a cost effective manner. The purpose of this paper is to develop a custom made, low power, cost-effective wireless sensor node based on a MSP430G2553 microprocessor and nRf24l01+ wireless transceiver module. Then, using these nodes, we form a simple WSN testbed, and locate the position of a light emitting source.

Parathyroid allotransplantation is increasingly practiced for patients who have permanent hypoparathyroidsm. Parathyroid allotransplantation success is varied, and no defined criteria about immunologic monitoring for pre-/post-transplantation follow-up. This study sought to evaluate the possible role of immunological tests. Four unrelated recipients and one living donor who have chronic kidney disease were evaluated for HLA-typing, PRA, C)CM tests to conduct parathyroid allotransplantation. Parathyroid glands were obtained and resected from the donor, then cells were isolated and cryopreserved. Upon histologic examination, cells wer . . .e cultivated and injected into muscle of four recipients. Recipient's were followed for parathormone and calcium levels for four years. PRA screening were monitored and de novo DSA was evaluated as well. In two of the recipients, allografts continued to be functional more than four years. In one recipient, allograft remained functional for two years and another recipient lost function after one year. Two out four were negative for de novo DSA and three out of four of the recipients remained negative for PRA. Neither HLA-matching nor de novo DSA positivity and PRA screenings seems significant for successful] parathyroid allotransplantation. This study has considerable potential for immunological monitoring of parathyroid allotransplantation Daha fazlası Daha az
