Association of interleukin 1beta gene (+3953) polymorphism and severity of endometriosis in Turkish women

Endometriosis is regarded as a complex trait, in which genetic and environmental factors contribute to the disease phenotype. We investigated whether the interleukin (IL) 1beta (+3953) polymorphism is associated with the severity of endometriosis. Diagnosis of endometriosis was made on the basis of laparoscopic findings. Stage of endometriosis was determined according to the Revised American Fertility Society classification. 118 women were enrolled in the study. 78 women didnot have endometriosis, 6 women had stage I, 3 had stage II, 13 had stage III and 18 had stage IV endometriosis. Polymerase Chain Reaction (PCR), Restriction Fra . . .gment Length Polymorphism (RFLP), and agarose gel electrophoresis techniques were used to determine the IL 1beta (+3953) genotype. Frequencies of the IL-1beta (+3953) genotypes in the control group were: CC, 0.397; TT, 0.115; CT, 0.487. Frequencies of the IL-1beta (+3953) genotypes in cases were: CC, 0.375; TT, 0.225; CT, 0.400. We found a 2.22 fold increase in TT genotype in the endometriosis group. However, the difference was not statistically significant (P > 0.05). We also observed an increase in the frequency of IL-1beta (+3953) T allele in the endometriosis group. However, the difference was not statistically significant. We also investigated the association between IL-1beta (+3953) polymorphism and the severity of endometriosis. The frequencies of CC+CT genotypes in stage I, III and IV endometriosis patients were 83.3, 84/6 and 72.2%, respectively; and TT genotypes were 16.7, 15.4 and 27.8%, respectively. We observed a statistically insignificant increase in TT genotype in stage IV endometriosis (P > 0.05). Psychologic influence of male factor infertility on men who are undergoing assisted reproductive treatment

OBJECTIVE: To investigate the psychologic impact of male factor infertility on men in couples undergoing assisted reproductive treatment in Turkey. STUDY DESIGN: A prospective study was carried out in a total of 105 men: 43 with male factor, 31 with female factor and 31 with unexplained infertility diagnoses. The men answered questionnaires-State Trait Anxiety Inventory, State Trait Anger Expression Inventory and Beck Depression Inventory-during treatment. The scores from each questionnaire were analyzed and compared according to the infertility diagnosis. RESULTS: No significant differences were found in measures of anxiety, anger . . .or depression among men with male factor, female factor and unexplained infertility. CONCLUSION: In the current preliminary study performed on a group of Turkish men who applied for in vitro fertilization treatment, it was found that male factor infertility as the cause of the couple's infertility problem did not have an adverse effect on the psychologic status of men undergoing assisted reproductive treatment. The effects of letrozole and melatonin on surgically induced endometriosis in a rat model: a preliminary study

Objective: To determine the effects of letrozole and melatonin on surgically induced endometriosis in a rat endometriosis model. Design: Prospective, randomized, controlled, experimental study. Setting: Experimental Research Center of Yeditepe University (YUDETAM). Animal(s): Thirty female, nonpregnant, nulligravid Wistar-Hannover albino rats. Intervention(s): Surgical induction of endometriosis, administration of estrogen for 2 weeks, and laparotomy; administration of letrozole or melatonin for 2 weeks after induction of endometriosis, and laparotomy; administration of estrogen for 2 weeks and necropsy. Main Outcome Measure(s): The . . . volume and histopathologic scores of endometriotic foci, and levels of superoxide dismutase, catalase, and malondialdehyde in the peritoneal fluid. Result(s): The mean volumes of the endometriotic foci were 99.6 ± 18.8 mm3, 21.5 ± 7.4 mm3, and 29.2 ± 17.5 mm3, and histopathologic scores were 2.5 ± 0.7, 2.0 ± 0.8, and 1.7 ± 0.9 in the melatonin group at the end of the second, fourth, and sixth weeks, respectively. The mean volumes of the endometriotic foci were 75.9 ± 26.3 mm3, 29.8 ± 14.7 mm3, and 121.2 ± 35.1 mm3 and the histopathologic scores were 2.5 ± 0.5, 2.2 ± 0.8, and 2.7 ± 0.4 in the letrozole group at the end of the second, fourth, and sixth weeks, respectively. In the melatonin group, peritoneal fluid superoxide dismutase and catalase levels increased statistically significantly. Conclusion(s): Melatonin caused more pronounced regression of endometriotic foci when compared with letrozole in a rat model. Preeclampsia complicates 10% of pregnancies in developing countries. It is one of the leading causes of maternal and fetal/neonatal mortality and morbidity worldwide. It has been suggested that maladaptation of the maternal immune response during pregnancy might be a causal factor for preeclampsia. According to immune maladaptation hypothesis, preeclampsia is due to an inappropriate regulation of normally Th2-deviated maternal immune responses, leading to a shift toward harmful Th1 immunity. Several studies indicate that monocyte chemotactic protein-1 (MCP-1) and CC chemokine receptor 2 (CCR2) are involved in Th1 and Th2 immunity. I . . .n this study, we investigated the association between MCP-1 A-2518G and CCR2-V64I polymorphisms and preeclampsia. One hundred eighty preeclamptic pregnant women and 145 healthy controls were included in the study. We observed that in preeclamptic women, MCP-1 G: CCR2 Val haplotype was significantly higher when compared with other haplotypes. Amaç: Gestasyonel diabet (GDM) tanısı koymak için 1 saatlik 50 gr glukoz tarama testinde (Glucose Challenge Test-GCT) bir eşik değer belirlemek. Materyal ve Metod: Bu retrospektif çalışmaya glukoz tarama testi pozitif çıkan 95 gebe dahil edildi. Bu gruba 1 hafta sonra 100 gr oral glukoz tolerans testi (OGTT) uygulandı. Test sonucuna gore diabet tanısı alan ve almayan hastaların verileri üzerinde kıyaslama yapıldı. Bulgular: Bu 95 gebenin 58'inde (%61.1) OGTT sonucu da pozitif olarak geldi ve GDM tanısı aldılar. Sonucu pozitif olduğu halde OGTT tahlili normal olanların oranı ise %38.9 (37/95) idi. Normal gebelerin GCT değer ortalamas . . .ı 155.89.3 mg/dl, GDM grubunun ise 165.814.7 mg/dl idi (p0.001). GCT eşik değeri 140 mg/dl alındığında sensitivite %86 iken spesifisite %19 e düşmekte idi. Eşik değer 200 mg/dl olarak kabul edildiğinde ise sensitivite %5, spesifisite ise %100 idi. Sonuç: GDM tanısı sadece GCT deki tek bir eşik değer göre konulamaz. Günümüzde hala GDM tanısının iki basamaklı GCT ve OGTT ile yapılması en uygun program gibi görülmektedir. Objective: To determine a cut-off value on the 50 gr glucose challenge test (GCT) for the diagnosis of gestational diabetes mellitus (GDM). Materials and Methods: Ninety five pregnant patients whose 50 gr GCT are positive were included in this retrospective trial. OGTT was administered after one week to these patients. The findings were evaluated based on the test results. Results: Fifty eigth of ninety five patients (61.1%) patients got GDM diagnosis according to their OGTT results. Thirty seven of this ninety five (38.9%) patients were accepted as a normal based on OGTT results. In normal group , the mean of the GCT results was 155.8±9.3 mg/dl, in GDM group, the mean of the GCT results was 165.8±14.7 mg/dl (p0.001). If the level of 140 mg/dl was accepted as a cut-off value for the diagnosis of GDM, sensitivity was 86% and spesifity was 19%. But when the cut-off level increased to 200 mg/dl, sensitivity decreased to 5%, spesifity increased to 100%. Conclusion: Diagnosis of GDM cannot be made just only with a certain cut-off level on GCT. It can be assumed a high incidence of chromosomal abnormality among the patients who underwent ICSI cycle after searching the current literature. The huge amount of these abnormalities consist of low grade mosaicism. Besides adequate follicular response, the rate of fertilization failure is high in these group. If pregnancy achieved, spontan abortion usually occurs. Oocyte donation can be most appropriate treatment choise in some selected patients.

Backround/Aims: Endometriosis is regarded as a complex disese, in which genetic and environmental factors contribute to the disease phenotype. Whether vascular endothelial growth factor (VEGF) -460 C/T and +405 G/C polymorphisms are associated with susceptibility to endometriosis was investigated. Patients and Methods: Diagnosis of endometriosis was made on the basis of laparoscopic findings. Stage of endometriosis was determined according to the Revised American Fertility Society classification. Sixty out of the 112 women enrolled had no endometriosis, 11 had mild or early-stage endometriosis and 41 had severe endometriosis. Polyme . . .rase chain reaction (PCR), restriction fragment length polymorphism and agarose gel electrophoresis techniques were used to determine the -460 C/T and +405 G/C genotypes. Results: The VEGF +405 G/C genotype frequencies among the cases and controls were CC 55.8% and 35%; GC 30.8% and 50.0%; GG 13.5% and 15.0%, respectively. The allelic frequencies were C 71.15% (cases) and 60.0% (controls) and G 28.8% (cases) and 40% (controls). Patients with endometriosis had a higher incidence of the VEGF +405 CC genotype compared with the controls (p=0.027). Women with VEGF +405 CC genotype had 2.3-fold higher risk for endometriosis. VEGF +405 GC genotype and G allele in the control group was higher than the endometriosis group (p=0.039, p=0.027 respectively). The VEGF -460 C/T genotype frequencies among the cases were CC 21.2%, CT 26.9% and TT 51.9%; the C and T allelic frequencies were 34.6% and 65.3%, respectively. The VEGF -460 genotype frequencies among the controls were CC 31.70%, CT18.3% and TT 50.0%; the C and T allelic frequencies were 40.8% and 59.1%, respectively (p>0.05). There was linkage disequilibrium between VEGF -460 C/T and +405 G/C polymorphisms (D': 0.197, r2=0.013). We observed that the VEGF 460T/405C haplotype frequency was significantly higher in patients compared to controls (p=0.011). Objective: To evaluate the outcome of women with hypogonadotropic hypogonadism (HH) undergoing in-vitro fertilization (IVF). Materials and Methods: Data from 13 cycles of ten hypogonadotropic patients treated with in vitro fertilization from the period January 2006 to January 2008 were analyzed and compared with treatment results from 20 patients with tubal factor infertility (TI). All patients underwent ovarian hyperstimulation for IVF/ICSI at the same center. HH patients initiated the treatment by receiving daily injections of hMG. The patients in the control group were given the same dosage of recombinant FSH. Results: Demographi . . .c characteristics of the patients were comparable. Mean duration of stimulation was 13 days in the HH group and nine days in the TI group; the difference was significant (p < 0.001). Significantly more gonadotropins were used for the stimulation of HH patients (p < 0.05). Peak serum E2 concentration was found to be higher in the TI group. We evaluated the proportion of metaphase II (MII) oocytes to total oocytes retrieved in HH patients and found the number was similar to the TI group. Despite that fertilization and implantation rates were similar in both groups, the cancellation rate was higher in the HH group (23.1% vs 0). However pregnancy and live birth rates were similar. Conclusions: The present study showed that HH women undergoing IVF/ICSI are good responders. Factors affecting sexual function in premenopausal age women with type 2 diabetes: A comprehensive study

Objective: To compare the factors affecting sexual function in premenopausal women with type 2 diabetes using the female sexual function index (FSFI). Design: Cross-sectional study. Setting: Premenopausal women with type 2 diabetes and healthy volunteers in an academic research environment. Patient(s): Sixty-two type 2 diabetic women were compared with 50 controls. The FSFI scores of the patients were compared according to age, duration of diabetes, marriage, socioeconomic status, parity, menstruation regularity, body mass index (BMI), and smoking. Intervention(s): None. Main Outcome Measure(s): Female sexual function index scores. . . .Result(s): The mean FSFI score in diabetics was lower than in controls. Menstrual irregularity and duration of diabetes affected female sexual function unlike age, BMI, socioeconomic status, duration of marriage, or parity. Regularly menstruating women had higher FSFI scores (27.1 ± 6.7) than irregularly menstruating women (20.7 ± 7.4). Type 2 diabetics were divided into three groups according to duration of diabetes. The women having diabetes longer than 10 years had the worse sexual function. Also, smoking affected inversely female sexual well being. Conclusion(s): Menstrual disturbance, smoking, and longer duration of diabetes (more than 10 years) impair FSFI scores in premenopausal women with type 2 diabetes. DNA repair genes in endometriosis

Several polymorphisms in the DNA repair gene are thought to have significant effects on cancer risk. We investigated the association of polymorphisms in the DNA repair genes XRCC1 Arg399Gln, XRCC3 Thr241Met, XPD Lys751Gln, XPG Asp1104His, APE1 Asp148Glu, and HOGG1 Ser326Cys with endometriosis risk. Genotypes were determined by PCR-RFLP assays in 52 patients with endometriosis and 101 age-matched healthy controls. Although there were no significant (P > 0.05) differences in the frequencies of genotypes or alleles of APE1, XRCC1, XPD, XPG, and HOGG1 genes between patients and controls, the frequency of the XRCC3 Thr/Thr genotype was s . . .ignificantly greater in endometriosis patients compared with controls (P = 0.005). XRCC3 Thr/Met genotypes (P = 0.022), and the Met allele (P = 0.005) seem to have a protective role against endometriosis. The distributions of genotypes and alleles of the genes APE1, XRCC1, XRCC3, XPD, XPG, and HOGG1 were not significantly associated with the different stages of endometriosis (P > 0.05).
